I would guess it should be at least 40 years, from the looks of her. Young artists sometimes have a twisted idea about older people. He probably was referring to the age of the song "Milkshake" by Kelis which was a hit in 2003. The woman in the comic still shouldn't look so old; see how Kelis looks now. I'm not so sure it refers to the song.
Girls "milkshakes" have been bringing boys to their yards forever.
Too old to remember the song and I thought the woman was the late Mrs. Barbara Bush. Some jokes just fly over my head these days!
