It's like in Doctor Who when a new character says, "Just what kind of doctor /are/ you?"
The ducks are for shooting at, though the suction-cup darts will only stick to the flat side. The gun is both pump-loader camouflaged as also a revolver. The microphone is for anime Nurse Ratched to announce it's time for everyone in the ward to take their pills (or suppositories; the big pill-things might be either). The EKG display belongs. And the bowling pins are for medicinal exercise (juggling) or maybe to pull the trigger on the gun with, because your finger won't fit in there.
With all that, there's room for an electrified nose-ring, indicated to the right of the masthead, but maybe they thought nobody would understand that, so why include it after all. I've only ever seen one in the real world.

Clearly the gun, bowling pins, etc. are all ways for a person to be injured. I think it would have made sense to me as a child.
