Years ago I worked for a company that handed out "anonymous" surveys. We noticed each form had a different number faintly written in pencil on the back. Most of the employees in my department either refused to answer the questions or made a photocopy of the form (without the numbers). At a later meeting we were told our answers were counter-productive and detrimental to the morale of the company. When asked about the numbers on the forms they denied any knowledge of them.
